The five documents are no longer required to submit, so you can send your profile now and return these later, either by resuming this form or by email to mentors@bartholdi-advisory.com. They remain contractual obligations and we will need them before your first engagement. The GDPR consents are still required. Banking proof is collected separately by secure link after submission.

What we need, and how to provide it. Your insurer issues this certificate, not Bartholdi. Ask them for proof of professional liability cover of at least one million euros. See an example certificate.

What we need, and how to provide it. We accept a passport or a national identity card, as a readable colour scan of the whole document. There is nothing to download here: this is your own document.
Banking details are handled separately.

GDPR consents
I consent to Bartholdi Labs storing the data submitted in this form for the purpose of operating my mentor relationship, including matching me to engagements, contracting, invoicing, and producing client materials that include my External Profile data.
I consent to Bartholdi Labs using AI systems to process my profile data for the purposes of mentor matching, automated production of mentor cards and proposal inserts, and generation of editorial content. AI processing respects the visibility tags applied to each field.
I consent to Bartholdi Labs using my External Profile content in editorial marketing pieces, social media posts, and the Bartholdi mentor wall. This consent is fully separate from the operational consent and may be withdrawn at any time.
